Do you have any restrictions or requirements due to COVID-19?
At this time, masks are no longer required for your service. Your provider will continue to wear a mask while mixing the IV bag. We do treat patients who are actively sick, including with COVID-19. If you are actively sick, we do ask that you wear a mask, in an effort to keep your provider and other clients healthy.
What forms of payment do you accept?
We accept all major debit and credit cards. We also offer payment plans with our financing partner, Cherry.
Why is my credit card information being requested at the time of booking? What is your cancellation policy?
Unfortunately, multiple last-minute cancellations have necessitated a change in policy. We now require that a credit/debit card be entered at the time of service. Your credit card is serving as a placeholder only.
We understand that cancellations happen. You have the right to cancel your appointment up to 8 hours before your scheduled appointment time without a fee. This will allow the time slot to be made available to someone else. If you fail to cancel your appointment at least 8 hours prior to your appointment or you fail to complete your intake paperwork at least 8 hours prior to your appointment, your appointment will be cancelled and you will be charged a $100 convenience fee. If you choose to reschedule, the fee will still be charged, but will be deducted from your service at the time of your rebooking.
Provided your appointment occurs as scheduled, you will not be charged until the time of the appointment itself, using one of the forms of payment above. We appreciate your adherence to and understanding of our cancellation and paperwork policies, so we can serve all of our clients as equitably and efficiently as possible.
Do you accept insurance or Medicare?
Rejuvenate MKE does not take insurance or Medicare. We apologize for the inconvenience this may cause.
You may opt to have your labs drawn at the lab of your choice, if you would like to use your insurance for this part of the process. However, we do not consult your insurance company to determine what, if any labs, will be covered by your plan. While the labs we request are fairly standard, Rejuvenate MKE is not responsible for any out-of-pocket costs incurred by your insurance company.
Do you accept HSA or FSA?
For the most part, most FSA and HSA plans qualify as a form of payment for many of our services, including medical weight loss, hormone replacement therapy, IV hydration therapy and functional medicine treatments. FSA and HSA plans do not cover aesthetics services. If you have a credit/debit card for your FSA or HSA plan, we can usually charge it directly. If required by your plan, we can provide you with an itemized Superbill which is a specialized receipt that includes the medical diagnosis and procedure codes applicable for your treatment. If you are unsure if your plan will reimburse you for your service, we encourage you to contact your plan provider directly.
Please note that Rejuvenate MKE is not responsible for claims rejected by FSA or HSA plans.
Are you available for large groups?
Absolutely! Consider utilizing services from Rejuvenate MKE before your bachelor or bachelorette party, corporate or pop-up event, group golf outing, etc. Discounts are available for group events; please contact us for more details.
Do you still offer mobile visits?
Yes, we do! However, moving into our clinic space will bring some changes to the mobile IV services we offer. Mobile IV services will still be available with the following changes:
- Mobile visits will be limited to two appointments per week.
- Mobile visits will have an additional convenience fee added. Clients within 30 miles of Oak Creek will have a $30 fee added to their service. Clients outside of 30 miles of Oak Creek will have a $50 fee added. The fee not per person, but per appointment. For example, if two people in your household are receiving a service, it is just ONE fee of $30 or $50, depending on your location.
- Groups of 4 or more will have the convenience fee waived.
- Our NAD+ infusions (Rewind, Rewind+, and Renew + Rewind) are only available in clinic.
Is there anyone who shouldn’t receive IV hydration therapy?
Generally speaking, IV hydration therapy is safe for most people. At Rejuvenate MKE, we do not offer our services to pregnant or pediatric clients, unless you have received clearance from your doctor. We will also ask questions about your general health and any allergies that you may have, that may make it unsafe for you to receive IV hydration therapy.
How often can I receive IV hydration therapy or an injection?
Typically, you can receive an IV infusion about every 2 weeks; after this time, you will likely find the effects you feel start to wear off. As for our injections, several of them can be safely given on a weekly basis, including B-complex and B-12, and NAD+ can safely be given twice a week.
Why do you ask for my medical history and require completion of intake paperwork?
At Rejuvenate MKE, we are a wellness clinic and we are performing medical procedures. Your health and safety is our utmost priority, so it is essential that we have a good understanding of your medications and medical history to ensure the treatments being performed are not unsafe for you. We understand this paperwork may be burdensome, but please rest assured it is requested with your best interest in mind.
