Membership Program

At Rejuvenate MKE, we appreciate the trust you put in us as your healthcare team to deliver you complete health and wellness. We understand that you could go anywhere for your services, and it means so much to us that you choose us!

We also understand that these services are a financial investment. To help make our services more affordable, Rejuvenate MKE offers memberships so you can invest in yourself.

Our memberships allow you to deposit money into a “wallet” that will accumulate as long as your membership is active with us. By being a member, you’ll be entitled to exclusive discounts on our products and services, which you can pay for using your wallet!

The Fine Print

Your monthly membership starts the first day of month after the month you signed up (unless you sign up exactly on the first of that month), and processes the first of each month thereafter. Your member discounts and benefits renew on that date as well. Your membership fee will be charged to your credit or debit card on file. 

Your health and wellness is a long-term investment. The minimum membership term is 6 months. In order to cancel a membership after the 6 month minimum term, a 30 day notice is required. To cancel a membership before the 6 month term is complete, a $500 fee is applied. Our typical member will stay with us for several years for maximum benefit to the member. 

Banking your monthly investment can be saved for as many months as you wish until you come in to redeem the saved monies. Banking your monthly investment can be saved up until the end of your 6 month commitment and it may be carried over by only three months. Funds that are accumulated in your wallet are only valid with an active membership.  If you choose to cancel your membership, you must spend the funds in your wallet prior to the end of your membership, or you forfeit these funds.  If you have referral credits, these cannot be used to pay for the cost of your monthly membership fee.  Membership discounts cannot be combined with monthly specials; members will be given the best discount at that time.

This is a member only benefit program and cannot be transferred to friends, family, or children in any manner. Our 24-hour cancellation policy still applies for all scheduled appointments and a $100 fee will be charged if this policy is not followed. If funds are insufficient and your payment cannot be processed, you will be required to update your credit card information within 48 hours.  If this happens two times or more during your participation in the membership program, you will be removed from it and no longer allowed to participate in it for the foreseeable future. 

Treatment prices are subject to change at any time and without notice. This may be due to unforeseen variables such as supply costs, demand and supply factors, market values, etc. Contracted discounts in the membership program are not subject to change and are locked in for a 6 month period from the original contract begin date.

*Filler discount is applied to an office visit. The multiple syringe discount is for multiple syringes used in the same office visit only.
